It is imperative that the Description and Address of the Commercial Unit and the Assessment Number is filled in on the form (hand written) as shown below.

This form is not to be completed for the following commercial property types:

  • Car Parks
  • Cycle Liveries
  • Gas Stations
  • Hotel Shops
