Lost or stolen passports

Lost and stolen application form (LS01) must be completed in addition to the passport application form. 

Lost or stolen passports can be reported to the police.

  1. 4 page single sided passport application form
  2. Must be in black ink
  3. Photo identification
  4. Sections required – 1, 2, 3, a, b (if applicable) and c, 4, 5, 8, 9, 10
  5. 2 photographs (1 must be countersigned)
    1. Section 6 is required if the child is age 12 – 15 years (note – if the child turns 12 years old within 2 weeks of submission, they must sign)
    2. Section 8 – tell us which countries passport your countersignatory holds and their email address (only for children 15 years and under)
    3. Section 9 – to be completed by the parent or the legal guardian under the age of 16 years. For a legal guardian(s) a court document is required.
    4. Section 10 – (note for applicants under 16 years - the countersignatory must confirm parent’s name)

Original documents (refer to notes) must be submitted for first time applicants, lost or stolen and name changes. Each application will be assessed on its own merit and may require further information.
