To support the health and safety of students in Bermuda with asthma, the Bermuda Department of Education, Department of Health, Bermuda Hospitals Board Asthma Education Centre, and Open Airways invite parents/guardians to complete the Bermuda Student Asthma Registry form. This initiative aims to ensure appropriate care for children (ages 4-18) managing asthma in schools.

Please Note: A separate form is required for each child with asthma. If your child does not have asthma, please exit the form.
